For each State/UT, collect dated official sources and separately record policy/guidance, service directory information, staffing or coverage data, evaluation findings and grievance routes. Define each indicator and its denominator before attempting comparison. No State/UT values are prefilled by this record.
LIMITATIONS
What it can—and cannot—tell us
This is a published method plan, not a completed matrix, map or ranking. Missing public information must not be treated as zero coverage or proof that a service is absent. Publish comparisons only after collecting values from comparable sources and documenting gaps.
